Important information about self-pay rates

Payment and price changes

Payment is due at the time of service. Prices shown reflect the discounted rate for patients paying in full at the time of service. Prices may be changed, modified, or discontinued at any time without prior notice.

Additional services, laboratory testing and medications

Some services may be charged in addition to other services provided during the same visit. In-office testing, injections and minor procedures are charged separately from the office visit fee. Laboratory testing is billed separately unless otherwise specified. Medications are not included unless otherwise specified.

Wellness, weight management, IV therapy and injections

Wellness, weight management, IV therapy and injection services are elective and are not covered by insurance. All IV therapy and injection services require a medical evaluation and a physician order before treatment.

Weight management and refunds

Weight management program fees are subject to the practice's written refund policy. Medication that has already been dispensed cannot be refunded. Refund requests for prepaid programs are handled according to the written refund policy and applicable Florida law.

Telehealth and telepsychiatry

Telehealth and telepsychiatry services require a signed telehealth consent on file before the first visit.

Good Faith Estimate — No Surprises Act

Patients who are uninsured or who choose not to use insurance have the right to receive a Good Faith Estimate of expected charges before scheduled services. You may request one in writing at any time. If you are billed at least $400 more than your Good Faith Estimate, you may dispute the bill within 120 days of the date on the bill. Estimates, itemized statements and self-pay rates

Upon request, the practice will provide a written, personalized estimate of anticipated charges before treatment. Patients may also request an itemized bill and explanation of charges. These self-pay rates apply only to patients who elect not to use insurance benefits for the service received. Medicare beneficiaries should speak with staff before electing self-pay.

Availability, actual charges and prescription products

This fee schedule is not insurance and does not guarantee that any particular service will be available or medically appropriate. Listed prices reflect standard, uncomplicated services; actual charges may vary based on visit complexity, services performed and clinical judgment. Prescription medications are dispensed only pursuant to a valid prescription following a medical evaluation.
